Healthy Check Up is not responsible for the circumvention of any privacy settings or security measures contained on the site by any users or third parties.

IP ADDRESSES

Healthy Check Up maintains internal logs for Internal Protocol (IP) addresses. An IP address is a number that is automatically assigned to your computer each time you access the Internet. Your IP address does not contain personally identifiable information. Healthy Check Up collects IP addresses in order to conduct system administration, report aggregate information, conduct site analysis, and to detect improper use of the site.

THIRD PARTY DISCLOSURE

We do not disclose information about you to nonaffiliated third parties, except as permitted by law. For example we are permitted by law to share information about you to agents, contractors, or affiliates of ours, who help us operate the website, conduct our business or help us complete a service you requested, such as processing your credit card payment. Healthy Check Up uses e-mail forms to allow our customers to ask questions about our services or programs. While we make every attempt to protect the personal information that you share with us, it is important to note that an e-mail message is unsecured. Should you choose to send your information via e-mail, you do so at your own risk. We recommend that if your communication is extremely sensitive you may want to send it by postal mail or through our online secure, Healthy Check Up Portal.
If you choose to correspond to us via e-mail, we reserve the right to retain the content of your email messages as well as your e-mail address or other PII, and our responses.

PERSONAL INFORMATION PROVIDED AS PART OF USER CONTENT

If you include personally identifiable information as part of any content that you provide to Healthy Check Up via public postings to the site, including any bulletin boards, forum posts, etc., such information may be made available to any parties using the site. Healthy Check Up does not edit or otherwise remove such information from content uploaded to any bulletin boards, forum posts, etc. before it is posted on the site. If you do not wish to have your personally identifiable information made available in this manner, you must remove any such information from the content before posting. Your contact information (which may include, without limitation, a username and email address) may be included with any content that you post via the site. Healthy Check Up is not liable for any damages caused or incurred due to personally identifiable information made available in the foregoing manner.

 

WHAT DO WE DO WITH YOUR INFORMATION?

When you purchase something from our store, as part of the buying and selling process, we collect the personal information you give us such as your name, address and email address.
When you browse our store, we also automatically receive your computer’s internet protocol (IP) address in order to provide us with information that helps us learn about your browser and operating system.
Email marketing (if applicable): With your permission, we may send you emails about our store, new products and other updates.


How do you get my consent?

SHOPIFY

Our store is hosted on Shopify Inc. They provide us with the online e-commerce platform that allows us to sell our products and services to you.
Your data is stored through Shopify’s data storage, databases and the general Shopify application. They store your data on a secure server behind a firewall.

Payment:
If you choose a direct payment gateway to complete your purchase, then Shopify stores your credit card data. It is encrypted through the Payment Card Industry Data Security Standard (PCI-DSS). Your purchase transaction data is stored only as long as is necessary to complete your purchase transaction. After that is complete, your purchase transaction information is deleted.
All direct payment gateways adhere to the standards set by PCI-DSS as managed by the PCI Security Standards Council, which is a joint effort of brands like Visa, MasterCard, American Express and Discover.
PCI-DSS requirements help ensure the secure handling of credit card information by our store and its service providers.
